Insurance Info
Payment Policy — Our office policy is to collect payment in full for services at the time of the visit. This includes all co-pays, deductibles, and prior balances on your account. For insurance plans with a deductible, we collect a "deductible deposit." We will bill your insurance company for you, and after we receive the "Explanation of Benefits" from your insurance company, we will bill you for any residual balance. Deductible deposits will continue to be collected at each visit until your deductible is satisfied based on information received from your insurance company. If you have any billing questions, please call our office at (623) 876-9983 and ask to speak to one of our billing specialists.

Insurance Information — We make every effort to verify your Insurance eligibility prior to you arriving. Please bring your current insurance card to every visit. We participate with most insurance plans; however, the verification of benefits is not a guarantee of payment for rendered services. We recommend that you call your insurance company to verify participation as well as your benefits as the plan member. When you schedule appointments, we do require you to provide us with all insurance information at that time. If we are unable to verify coverage with your insurance company, you will be considered a private pay.
