Payment Policy: Our office policy is to collect payment in full for services at the time of the visit. This includes all co-pays, deductibles, and prior balances on your account. For insurance plans with a deductible, we collect a “deductible deposit”. We will bill your insurance company for you, and after we receive the “Explanation of Benefits” from your insurance company, we will bill you for any residual balance. Deductible deposits will continue to be collected at each visit until your deductible is satisfied based on information received from your insurance company. We accept cash, checks, Visa, and MasterCard. Insurance Information: If we are contracted with your insurance, we will submit claims. Please bring your current insurance card to every visit. We participate with most insurance plans; however, we recommend that you call your insurance company to verify participation as well as your benefits. When you schedule appointments, we do require you to provide us with all insurance information at that time. We make every effort to verify your eligibility prior to you arriving. If we are unable to verify coverage with your insurance company, you will be considered a private pay.
We also accept most PPO networks. Call our office for more information, even if you do not see your insurance company on this list. This list will not always be complete as changes with companies occur often.
