If you would prefer an appointment for a sick child rather than leaving a telephone message for advice, we ask that you call us in the morning for a same-day sick appointment. It is best to always schedule out your well-child appointments at least two (2) weeks in advance to ensure you get the provider and time that you prefer.
If you are a new patient, we must have the parent or legal guardian bring the child to that first appointment. In our patient registration forms, we ask you to grant permission to those you wish to be allowed to bring your child after that. At each appointment, we will also ask you to bring your insurance card, as well as a photo ID that includes your current address. You are also advised to bring the child’s shot record to every well-child check. Please keep in mind we still ask whoever brings the child to the appointment to abide by our financial policy and to show ID. In addition, we do not accept third party assignment nor do we recognize or enforce the terms of divorce decrees.
It is also our physician’s strong belief that all children should get appropriately scheduled well-child exams. Because of this we will not vaccinate kids without doing a complete well-child examination.
